DEATH RECORD

Winnebago County issues only certified copies of death records, which are acceptable for legal identification purposes.

HOW DO I OBTAIN A CERTIFIED COPY OF A WINNEBAGO COUNTY DEATH RECORD?

Deaths that occurred in Winnebago County are located in the Winnebago County Clerk's office. You can either come into our office and purchase a certified copy of a death record or send us a request through the mail.

The only individuals who can purchase a complete, detailed death record are the immediate family or a court appointed representative. A certification of death can be purchased by anyone.

If you come into our office:

Bring a valid drivers license for identification. *Note: If you do not have a valid driver’s license, see list of acceptable identification listed below. The cost is $17.00 for the first certified copy and $6.00 for each additional copy. You will be asked to fill out a form. The whole process takes approximately 15 minutes. The Winnebago County Clerk’s office is located in the Winnebago County Administration Building 404 Elm Street, on the first floor, Room 104, Rockford, IL 61101. We are on the corner of Elm and South Church St across from the MetroCentre.

Request via the mail:

Make a copy of your valid driver’s license. *Note: If you do not have a valid driver’s license, see list of acceptable identification listed below. On the copy write deceased’s name, date of death, your relationship to the person whose death record you are requesting, and your return address and phone number with area code. Attach a check or money order payable to: Winnebago County Clerk. DO NOT SEND CASH! The cost is $17.00 for the first certified copy and $6.00 for each additional copy. Mail the copy of your ID, the requested information and the fees to the following address: Winnebago County Clerk, 404 Elm Street, Room 104, Rockford, IL 61101. If we receive all of the requested information, we will mail you the death record within 2 business days of receipt.

*LIST OF ACCEPTABLE IDENTIFICATION NEEDED TO RECEIVE CERTIFIED COPIES OF BIRTH, MARRIAGE, AND/OR DEATH RECORDS:

*ONE COPY OF IDENTIFICATION FROM THE FOLLOWING LIST......

  • Certified Copy of Birth Certificate of person requesting the document
  • VALID Illinois Driver's License
  • VALID Illinois State Identification Card
  • VALID Out-of-State Drivers License or State ID Card
  • U.S. Government Driver's License
  • Naturalization Certificate
  • U.S. Military Identification Card
  • U.S. Immigration Card
  • Selective Service Card
  • Illinois Public Aid ID Card with Medical Card

*OR COPIES OF TWO DIFFERENT PIECES OF IDENTIFICATION FROM THE FOLLOWING LIST.......

  • Social Security Card or Print-out
  • Voter Registration Card
  • W-2 Form
  • Bank Statement (within 90 days)
  • Utility Bill (within 60 days)
  • Vehicle Registration Card
  • Active Insurance Policy (at least one year old)
  • Checkbook or Savings Book (showing name & address)
  • Employee Identification Card
  • Traffic Ticket
  • Resident Alien Card
  • School Identification Card

All documents must be current and valid. The Winnebago County Clerk's Office Reserves the right to request additional identification to receive records. For Mail-in requests, copy(s) of identification must be included with the request.
